Confidentiality & Medical Records
This Practice wants to ensure the highest standard of medical care for our patients.
We understand that a General Practice is a trusted community governed by an ethic of privacy and confidentiality. Our practices are consistent with the Medical Council guidelines and the privacy principles of the Data Protection Acts.
We see our patients’ consent as being the key factor in dealing with their health information.
This leaflet is about making consent meaningful by advising you of our policies and practices on dealing with your medical information.
Reception and administration staff require access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.

Notification Consent Form
Patients may consent to the practice contacting them by text message and email for the purpose of receiving appointment reminders and notifications that test results are available.

Data Protection Consent Form
The personal information collected from patients will be held by the practice in electronic format on personal medical records.
The purpose of holding this information is for the provision of appropriate healthcare, treatment and services, and to ensure continuity of care and patient safety.
The information will be processed in accordance with Data Protection legislation. Disclosure of this information will only take place with express consent or in accordance with legislation or regulatory requirements.

Complaints
We make every effort to give the best service possible to everyone who attends our practice.
However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible.
To pursue a complaint please contact reception or Dr. McGrath who will deal with your concerns appropriately. Further written information is available regarding the complaints procedure from reception.

Unacceptable Behaviour
It is our policy to treat everyone with respect and courtesy and we expect the same respect and courtesy in return. Any aggressive or inappropriate behaviour towards staff will not be tolerated and will be dealt with seriously.
Interruptions
We try not to have interruptions during patients’ appointments. If you need to speak to the doctor or nurse please leave a message with the reception and we will return your call when the surgery is finished. If you have an urgent enquiry that cannot wait please tell reception.
